We apologize, but at this time we are no longer taking new Medicare or Medicaid patients except for non-HMO Medicaid and VA Premier which is listed above. Other private plans will be billed for the patient's convenience but you will be responsible for the balance of what your insurance does not cover.

How can I help support FCHC and their mission?
As a non-profit organization, we are always looking for donors. We receive grant money through various organizations, but are blessed by those who would like to support our mission. Please call the office to discuss how might help us meet any of the specific needs of the office. We thank you for any contribution that you make!
What if I don't have insurance?
It's ok, you can be a patient at our office. FCHC offers a sliding payment scale for those who need assistance.
Do I need a referral?
If FCHC is your primary care physician, then you should not need a referral. Sometimes you have to first list who your primary care physician is with your insurance carrier before your first visit. You should be able to find out if you need to do that by calling the Customer Service number on your insurance card.
